Terms & Conditions *I agreeIn consideration of participation in this Tennis Clinic for myself/ourselves and as parent, next friend, or guardian do forever release, acquit, and discharge the two coaches, Mercurio Evangelista and Chuck Radda, from demands, actions and causes of action which I/we or my/our representative may have by reason of an accident, injuries, or losses occurring during the participation in the organized recreation program named above. As further consideration of participation in the above named program, I/we agree to indemnify and save harmless the said coaches against any and all future claims for damages, costs, and expenses by or on behalf of the child arising out of participating in said program. NOTICE: In spite of every precaution taken by said coaches, there is always a chance your sons and daughters may be injured or hurt participating in the above named activity, and we want you to understand that.
